3.4.4  Check Request Database Form

Topics Covered:

  • Good form d
  • Project accounting
  • Accounts payable
  • 4th Dimension solutions

Business Objectives:

The most frequently used forms at SCP Communications, a medical commuications company, is the check request form — thousands of these are printed and processed each year. Most of these forms are generated automatically by TRAKit!, SCP's primary production database system.

The original form design was cluttered, and the order of the fields on the form slowed data entry into SCP's accounting system. The form contained fields which were no longer used, and did not contain other fields which were required. Busy project managers frequently filled in the missing information incorrectly which threw off the cost-accounting systems and severely hampered project reconciliations.

How Objectives Were Met:

The form was redesigned to give a contemporary, professional appearance consistent with SCP's corporate design standards. Graphics on the new form are high resolution and print cleanly.

Obsolete fields were removed from the form, and the remaining fields were reordered to streamline data entry by accounting. Missing fields were added to the form and filled TRAKit! ensures that the values are accurate. Additional fields were added to aid in after-the-fact research by project accountants and account managers.
